Registration Waiver:
By registering for this event, I or for minor my child, do hereby voluntarily submit my application for attendance and participation in said seminar and do hereby assume full responsibility for any and all damages, injuries, or losses that I may sustain or incur, if any, while attending or participating in said event I hereby waive all claims against the Martial Arts Collective Society, Martial Arts Center International, E.M.A.C.,LLC, Bushin Warrior Gear, UWTAF LLC., Bruce Juchnik, Sei Kosho Shorei Kai International, Circus Circus Hotel, Reno, the School Owners, instructors, officials, competitors and students associated with said event, individually or otherwise, for any claims for injuries that I might sustain. I fully understand that any medical treatment given to me will be of a First Aid type only. I consent that any pictures or videotape furnished by me or taken of me in connection with said event can be used for publication, promotion, and media coverage, and I agree to waive any compensation for the use of pictures, videotape, media coverage, statements, interviews, etc. by those producing this event.
If under the age of 18, this release and consent to participate is agreed upon by parent or person responsible for said minor.
No refunds will be provided within 7 days of the event.
